1. Two application forms both duly signed by the applicant.
   (Photocopies of the application accepted)
2. Two recently taken photographs, (color or black & white)
3. Valid passport to travel
4. A letter from applicant's employer confirming that the a[pplicant will return to the United States of America to resume his/her duties after the visit or A letter from air line or the travel organization guaranteeing onward return air ticket.
5. A single rate of $131 U.S. is applied for long/short stay visa
6. If applying by mail, please enclose self addressed stamped envelope for the return mail. EXPRESS MAIL ONLY.
7. Yellow fever immunization required (Only yellow fever)
