Two application forms both duly signed by the applicant.

(Photocopies of the application accepted)

Two recently taken photographs, (color or black & white)

c)  Valid passport to travel

d)  A letter from applicant’s employer confirming that the a[pplicant will return to the United States of America to resume his/her duties after the visit or A letter from air line or the travel organization guaranteeing onward return air ticket.

e)  A single rate of $150 U.S. is applied for long/short stay visa

f) If applying by mail, please enclose self addressed stamped envelope for the return mail. EXPRESS MAIL ONLY.

g) Yellow fever immunization required (Only yellow fever)
